Indiana Wesleyan University utilizes a third-party company, Med+Proctor for our Health Form and Immunization reporting.
There is no charge to use this service, though you have the option to purchase a membership which will provide lifetime access to your immunization data that can be shared with others, as you designate.
Using Med+Proctor is a quick, and easy way to provide IWU with the necessary health forms prior to starting school.
Visit medproctor.com and select register to start a new account.
Type in your IWU student email address to get started and follow the directions to register a new account.
Fill out any required personal, medical or insurance information.
Then, download the required forms and follow the directions provided.
Log back in to your Med+Proctor account and upload a copy of your forms.
Make sure your forms are complete and legible. You will receive an email confirmation once the forms have been reviewed.
Med+Proctor offers a chat line from 9 a.m.-9 p.m. daily.
Visit https://www.medproctor.com/student-help to read articles that may address your questions or use the chat line (you will see it in the lower right-hand corner of their website).
There is no cost to use the Med+Proctor website.
However, there is a $10.00 fee option for lifetime access to your health records stored on Med+Proctor.
If you do not want lifetime access, click “Continue”; there is no fee for Med+Proctor services needed for IWU's health form requirements.
You may simply take a picture of the immunization form and upload through your mobile device. Or, you may save a scanned image of the form to your computer and upload.
Med+Proctor will not accept a word document.
