Incident reports are an important mechanism to record the details of an event and to prompt a review by the appropriate University officials. Use the following table to determine the proper report.
Most forms allow for anonymous reporting; however, the ability to adequately respond to the incident may be hindered.
For emergencies, individuals should call 911 (or the country equivalent) or Campus Police at +1 (765) 677-4911.

All non-employee incidents, including students, visitors, contractors / vendors, and volunteers. |

Employees, including student employees. Includes work-related injuries, illnesses, medical incidents, exposures, property damage, accidents, environmental incidents, near misses / dangerous activities, and equipment malfunctions. |

Instances of sexual harassment, sexual assault, rape (including statutory), dating violence, domestic violence, stalking, or other related sexual misconduct involving members of the IWU community. |

Theft, embezzlement, fraudulent activities, conflicts of interest, abuse, ethical violations, misconduct, falsification of records, and other policy violations within the IWU community. |
|
Crisis situations with IWU travelers working, serving, and learning outside of the United States. |
|
Any individual who experiences, witnesses, or learns of potential bias situations within the IWU community involving, but not limited to, race, ethnicity / national origin, gender / sex, disability, religion, age, veteran status, genetic information, sexual orientation, and acts of retaliation. |

Instances of cheating, plagiarism, falsification, collusion, and academic misconduct involving IWU-Marion students. |
|
General dissatisfaction with some aspect of IWU. |

Student complaints that were not able to be resolved through existing IWU processes. |
